
MAP Admission
The Multisensory Academy of Practitioners (MAP) provides a distinctive opportunity for students in grades seven through 12 who desire a multisensory and individualized educational experience in conjunction with an active and positive social life. The Forsyth Country Day School (FCDS) college preparatory core curriculum is implemented using various multisensory methodologies in classes featuring no more than three students and is supplemented by unique elective course offerings. Students within the MAP program, with various types of learning differences and average to superior intelligence levels, take advantage of top quality educators and extensive use of assistive technology. Students in the MAP program also enjoy opportunities to participate with their peers at FCDS in morning meetings, breaks, meals, clubs, and all co-curricular activities.

MAP Admission Procedures
- A family schedules a personal visit with the MAP Director and takes a tour of the combined MAP – FCDS campus.
- An application packet is received following an initial visit or upon request by the family.
- Upon receipt of the completed application packet and application fee (see ADMISSION REQUIREMENTS CHECKLIST below), the MAP Director establishes a file on the student.
- The applicant's information is reviewed by the MAP Admissions Committee, and the MAP Director then informs the applicant and his/her family of the admission decision.
- If admission is not granted, the MAP Director will help explore other options;
- If admission is granted, the signed enrollment agreement and the enrollment deposit are returned to the Business Office and an account is established for the applicant.
- If families wish, they may review the Financial Aid Procedures.
- Each student then meets with the MAP Director (and appropriate practitioners) to establish a class schedule. An ATLAS, used to chart each student’s progress, stay organized, and maintain open and effective communication with practitioners, parents, peers, will also be initiated.
MAP Admission Requirements Checklist
The following must be completed and/or received by the Director prior to consideration:
- A MAP Application Form completed by the parent(s)/guardian(s) of the applicant
- A MAP parent Statement completed by the applicant’s parent(s)/guardian(s)
- A MAP Student Statement completed by the applicant
- A MAP transcript and information release Form completed by the parent(s)/guardian(s) of the applicant for each MAP Teacher observation/recommendation Form to be completed
- At least two (2) MAP Teacher observation/recommendation Forms completed by current (within the past one year) teachers. Math and English teachers must each complete one of these forms and more than two (2) forms may be completed if desired.
- A recent (within the previous one year) transcript from previous educational institution.
- A psychological/educational evaluation meeting these minimum requirements:
- must include a diagnosis of a specific learning disability
- must be current – within the previous three (3) years
- must include relevant educational, developmental, and medical history
- must describe the comprehensive testing and techniques: include subtest scores
- must describe the functional limitations to learning
- must describe specific accommodations
- must establish the professional credentials of the evaluator
Examples of suitable testing instruments include:
- Woodcock Johnson Psycho-educational Battery
- Wechsler Intelligence Scales for Children and/or adults
- include tests of achievement and cognitive abilities
- Test of written language
- Wechsler individual achievement test
- Any recent (within the previous three years) disciplinary record(s) pertaining to in-school and/or out-of-school behavior.
- A non-refundable application fee of $200
